Forms and Documentation  

When you enrol your child, you will need to complete an enrolment form and hand this into the school's office. The enrolment form can be collected from the school's office or click on the button below:

You will also need to bring:

• An immunisation certificate (available from your doctor).
• If your child was born in New Zealand - a copy of the child's birth certificate or passport.
• If your child was NOT born in New Zealand, please bring the following documentation: 

1. Student/Work Visa, OR
2. Permanent Residence documentation, AND
3. Passports of all family members (Child, Mother, Father)

On arrival at the office we also ask for you to fill in some other forms such as  privacy/consent documentation regarding photo and video publishing and a student digital user agreement. Please let us know if your child will use our bus services. We will need to confirm with your address if your child is eligible to make use of this service.
